How to Remove Your Donor Designation
If you are registered as an organ and tissue donor on your driver’s license through your state department of motor vehicles, you can remove your donor designation in these ways:
Connecticut
You can modify your donor registry status on the CT DMV’s Donor Registry online, by appointment at a DMV service location, or by mail with the CT DMV’s Change of Address/Donor Registry Status Form. All options are outlined on the CT DMV’s website.
Maine
Visit the BMV organ donation page HERE. From there, visit the link to the Secretary of State’s Organ Donor Registry Online Service, where you can remove the donor designation from your driver’s license record.
Massachusetts
One can easily remove oneself from the RMV organ donor registry online by accessing your state RMV information.
Go to: https://atlas-myrmv.massdot.state.ma.us/myrmv/_/
And then go to “Access My Profile”
Plug in your: Driver’s License number, Date of Birth, last name, and last 4 of your SSN
Once logged in, go under the “Class D License” section and on the far right of that box at the bottom of the list, click “more”
From there, the 5th option down is “Change My Organ and Tissue Donation Status”
From there, you can change your status
New Hampshire
One can remove themselves from the organ and tissue donation registry by filling out the DSMV 30 – NH’s Record Change Request form and checking off the box that they want to be removed. Then one can mail it in or drop it into a drop box at their nearest NH DMV office. That Record Change Request form can be found here: www.dmv.nh.gov/sites/g/files/ehbemt416/files/inline-documents/dsmv30.pdf
Rhode Island
One can change their organ donation status online on the RI DMV site by requesting a duplicate and checking NO to the Organ and Tissue Donor question. It can also be done in person or by mail by filling out the LI-1 form.

National Donate Life Registry
If you registered yourself directly with the National Donate Life Registry online, you may access your registration here and remove your designation
